2011 Registration Information

Registration fee is $45.00 for 1st child and $35 for others.

Each player will be required to sell 5 - $5.00 raffle tickets

(payable in advance at sign ups!).

Selling space in our patron book is available.

New players will need a picture of self and a copy of their birth certificate.

· Adult conduct code form

· Jersey form  (if needed)
